Facility Evaluation Report

LPA Nune Margaryan conducted an unannounced case management visit to investigate the incident reported by Facility Administrator on 08/05/25. LPA met with House Manager. Administrator arrived shortly after. LPA explained the reason for visit. The department received SIR and SOC341 on 08/05/25 regarding Client 1 (C1) physically was abused by Staff 1 (S1).

The Incident report indicated that S1 poured a pitcher of cold water on C1 as an intervention to a behavior while on shift. LPA was advised S1 was put on suspension pending investigation on 08/04/25 and was terminated on 08/07/25. Copy of Termination letter was provided to LPA. It says that S1 confirmed their statement that they "put water in the jug and poured on client to calm down" and "Due to her admission, employee is terminated for failure to follow appropriate behavioral intervention techniques based on CPI training and the client's behavior management plan. Such actions are considered a Major Violation of Company Policy".

Azusa PD was notified and Police Officer, Butch #1224 conducted visit on 08/05/25. No Police report was filed due to no crime and injuries and Claim number was provided: Claim # 25-27562.

During today’s visit LPA toured the facility, Interviewed Administrator, and House Manager. C1 and C2 were at the Day program at the time of visit and C3 who was present at the time of visit refused to be interviewed. No health and safety issues observed at the facility.
