Facility Evaluation Report
Licensing Program Analyst (LPA) Nune Margaryan conducted an annual required visit. LPA met with Licensee / Administrator, Jesusa Viloria. The purpose of the visit was explained. The facility is licensed to serve (6) mentally disabled adults ages 18 - 59. There are currently 3 ambulatory clients residing at the facility. At the time of visit one client was at home, second client was out in home visit and third client was at the day program.
LPA toured the home and inspected 2 client bedrooms, (1) family room (3rd clients room temporary used as a family room), (1) staff bedroom, (1) client bathroom, (1) staff bathroom, kitchen, dining / living room, office area, laundry area which located in the attached garage. The front and backyard are well maintained and there are no pools or large bodies of water. There is a shaded seating area for the clients located in the backyard. Passageways and exits are free of obstruction. The water temperature was tested in the client’s bathroom and measured at 117.3 F which is within the required 105 - 120 degrees. Client bedrooms have the required furniture such as bed frames, dressers, lamps, and chairs. Bedrooms also have sufficient closet space. Clients’ beds have the required linen, and the linen is in good condition. Extra linens, blankets, towels, and personal hygiene supplies were observed in the closet located in the hallway. Smoke detectors were observed throughout the facility. There is a carbon monoxide detector in the hallway and was tested and operable during the visit. There is a fireplace located in the living room area which covered by a screen. There is a fire extinguisher located in the kitchen and it is fully charged. Kitchen appliances are clean and were operating at the time of the visit. Second refrigerator observed in the garage. LPA observed an appropriate food supply of nonperishable foods for a minimum of one week and perishable foods for a minimum of two days.
