Facility Evaluation Report
On 3/8/2024 at 8:18 a.m., Licensing Program Analyst (LPA) Jewel baptiste conducted an annual required inspection at Founders House of Hope. Upon arrival LPA met with Assistant Administrator Erlinda Ramos and explained the reason for the visit. The Assistant Administrator contacted the Administrator Honeylet Urrea and informed them of the visit. LPA used the care tools to evaluate the facility.The licensee prefers to serve mentally disabled adults aged 18-59 years ambulatory only. The last emergency disaster/fire drill was conducted on 2/21/2024. The Administrator certificate expired on 10/11/2023 #6044789735. LPA observed the Administrator Certificate on the pending list on Community Care Licensing website. During today's visit LPA inspected the physical plant inside and outside, reviewed the food supply, tested the smoke/carbon monoxide detectors, reviewed (4) staff files, (5) client files, medications, and medication administration records for (5) clients and P&I. Interviews was also conducted with 5 clients and 4 staff.
The facility is a two-story residence which consists of 51 client rooms. 35 client’s rooms are downstairs, and 12 client’s rooms are upstairs. There are 36 bathrooms downstairs and 13 bathrooms upstairs. The showers are jack and jill showers and on each side of the showers there are clients of the same sex. Downstairs consists of a reception/ staff office, activity room, medication room, dining area, kitchen, store, laundry room and 3 patio and 1 parking lot with smoking area. Upstairs consists of pool room, conference room, electrical room, employee break room and Administrator office. LPA toured the facility with the Assistant administrator and facility coordinator. LPA toured Rm#1, #6, #8, #17, #27, #36A, #39, #42, #43 and #46. Each client’s room has a smoke detector, bed, linen, dresser, light, and sufficient closet space. Hot water temperature was measured between 107.6 degrees and 117.6 degrees F. LPA toured the kitchen and noted working appliances; refrigerator, stove, oven and contained dishware, cups, plates, utensils, pots, and pans. The pantry was well stocked with canned goods, pasta, cereals, and the food supply contained a sufficient supply with a two-day supply of perishables and a seven-day supply of non-perishables that met title 22 guidelines. All the cleaning supplies and sharp utensils are locked and inaccessible to clients. The client’s medications are centrally stored and locked in the medication room. LPA inspected the smoke detectors and carbon monoxide detectors, and they are operated properly. (Report continued on 9099c)
