Complaint Investigation Report
S1 stated facility staff is here to assist clients with their laundry of personal clothing but clients are responsible to bring it to the laundry room and place it in the washing machine and take out of the washing machine and place it in the dryer and take it back to their rooms. Laundry assistant stated that resident's do their own laundry with some assistance if needed like providing them with detergent or placing the laundry in the washing machine or dryer. Facility will assist clients that cannot do their own laundry but will not allow family or others use the laundry facilities. The facility does full laundry service for linens, towels and sheets. LPA toured several rooms randomly and did not observed any bed sheets with holes or uncleaned sheets including C1
The facility has doctor that tends to clients needs in house and brings a mobile vehicle that accords privacy, also, the facility has rooms in facility that provide privacy to clients in the facility. LPA interviewed five staff and all five staff denied the allegations and stated they provide all clients needs and privacy. LPA interviewed nine clients and 8/9 could not collaborate the allegations and 8/9 stated that all their needs are being met. C1 refused to answer questions.
Regarding Allegation: Staff forced resident to change to medical provider . It is alleged that facility staff forced client to change to in house doctor as condition to remain at facility. Administrator stated that clients are encouraged to use in house doctor for client's convenience but never are clients force to change their medical provider. Administrator stated that C1 medical provider is managed my C1 conservator and no change has been made in that regard. LPA interviewed 5 staff and all 5 staff denied that allegation and stated that no client is forced to do anything at the facility including changing medical provider. LPA interviewed 9 clients and 8 of 9 could not collaborate the allegations and all 8 stating they have never been asked by facility to changed medical provider and are satisfied with arrangements at facility. C1 refused to answer questions. Conservator (W1) for C1 stated that W1 has not given up C1 right to choose a medical provider and C1 was not forced to change medical provider.
Regarding Allegation: Staff did not safeguard resident's personal belongings. It is alleged that resident is missing 2 articles of clothing. W1 could not identify article of clothing that C1 is missing. C1 refused to answer questions. LPA interviewed 5 staff and 5/5 denied the allegations. LPA interviewed 9 clients and 6/8 stated they have never lost any personal property or clothing while at facility.
Based on LPA's observation, interviews, and file review the investigation revealed: Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations are UNSUBSTANTIATED. Exit interview conducted with Administrator Crysel Santos and a copy of this record provided.
