The inspector’s account

Licensing Program Analyst (LPA) Sakinah Madyun conducted an unannounced annual inspection visit at the facility using the CARE inspection tool. LPA met with Edward Cruz and explained the reason of the visit.

The Adult Residential Facility serves three (3) ambulatory only clients in the age range of 18 through 59. The fire clearance has been approved for three (3) ambulatory clients. This property is currently licensed since 09/09/2020. The physical plant was toured with the Administrator Edward Cruz approximately at 1:40pm.

The facility is in a residential area and is a one-story family home. There are two (2) client bedrooms in which one is shared, one (1) receiving room, one (1) bonus room, one and half (1 1/2) bathrooms, living room, dining room, kitchen, front yard, back yard, garage/staff room/office.

LPA conducted the tour and observed the following: Smoke detectors were observed in living room area and in each client’s bedroom. There is one (1) carbon monoxide detector in living room. Client’s bedrooms were observed to have required furniture such as bed frames, dressers, chairs, lamps, and sufficient drawer space and the required bedding was also observed. The home has one and half (1 1/2) bathrooms that were clean and sanitary. The water temperature was tested and measured at 111.2-118.2 degrees F, which is within the required 105-120 degrees F. There were two (2) fully charged fire extinguishers observed in the receiving room and kitchen. All appliances in the kitchen were observed to be operational. The sharp knives are located in a kitchen drawer locked and inaccessible to clients. Cleaning supplies and chemicals are stored in a locked cabinet in the garage/storage area. No medications are being centrally stored due to the facility not having clients housed at this time. No files have yet been created due to no clients being housed. All files will be kept in the office in the future once clients are housed. The garage will be utilized for staff room and storage to secure chemicals. Continue LIC 809C
