Licensing Program Analyst (LPA) Galarza initiated a Case Management- Deficiencies visit due to record review and interviews conducted while investigating complaint control #: 28-AS-20250516111706. The purpose of the visit was explained to Program Manager Stephanie Smith.

Former Administrator Ann Hyde's last date of employment was August 28, 2025. The Regional Office did not receive notification of the personnel change. As of today, change of Administrator documents have not been su bmitted to Community Care Licensing.

Per 85061 (b) Reporting Requirements. The licensee shall notify the licensing agency, in writing, within 30 days of a change of administrator. Such notification shall include the following:
