The inspector’s account

Licensing Program Analyst (LPA) Alberto Lopez made announced visit to finalized facility closure. LPA took tour of facility and inspected every room. LPA met with Program Manager Sheleania Hampton and discussed the purpose of the visit.

LPA did not observe any residents living in the facility and staff stated all residents have been relocated.

LPA did not observe any health or safety risk.

LPA secured License and removed it from home. .

Exit interview conducted and copy of report provided. .
