Facility Evaluation Report
Licensing Program Analyst (LPA) Mariana Agban conducted an announced Annual Required visit and inspection of the facility. The facility currently has zero residents. LPA met with Administrator Lilit Hovakimyan, and at approximately 10:00 am, with the Administrator's assistance, LPA took a tour of the physical plant. Required postings were observed in the entry area. LPA tested the smoke alarms and carbon monoxide detectors, and they were functional. LPA observed a fire extinguisher with the date of purchase 02/19/26. LPA observed the kitchen to be clean, and the kitchen appliances are functional. Sharp objects were stored in a locked cabinet. LPA found a sufficient amount of perishable and non-perishable food at the facility, properly stored. Common Areas: These included the living room and dining area. The common areas were properly furnished. The dining room table is large enough to seat the capacity of the facility. Seating, such as couches, where in good repair and seats the capacity of the facility.
Bedrooms: There are four (4) bedrooms in the facility. All four (4) bedrooms are designated for shared use. All bedrooms were furnished with appropriate bedding and linens and had sufficient lighting.
Bathrooms: There are three(3) bathrooms designated for residents' use. The bathrooms were observed to have the proper fixtures, grab bars, and non-skid mats. Hot water temperature was measured from the bathroom sink at 106.5 and 107.9 degrees Fahrenheit. No cleaning supplies or hazardous items were present in each bathroom during the inspection. LPA observed the washer and dryer appeared to be functional. All toxins are locked in the hallway cabinet.
Surrounding Grounds: All entry and exit doors have a functional auditory alert when the doors open. LPA observed the front yard furniture that accommodates six (6) residents. The facility has sufficient yard space. No pools or bodies of water observed in the facility.
