Facility Evaluation Report
On 07/21/25 at 9:45AM, Licensing Program Analyst (LPA) Gina Saucedo, arrived to conduct an unannounced, annual inspection at the facility. Upon arrival, LPA Saucedo met with Marine Hakobyan and disclosed the purpose of the visit.
There is one entrance being utilized directly in front of the facility. The facility has a total of four (4) bedrooms and four (4) bathrooms. The facility is fire cleared for four (4) ambulatory only. The facility is currently occupying four (4) ambulatory residents and it is a level iii (3) facility.
Outside/Backyard: The facility has outdoor furniture with a covered shaded area for residents, visitors and staff and a small television. The facility does not have a swimming pool/bodies of water.
Kitchen is sufficiently stocked with at least seven (07) days perishable and seven (07) days non-perishable food. Frozen foods are wrapped and stored appropriately. Knives/sharp s /chemicals/toxins are observed to be locked and inaccessible to residents and are kept under the kitchen sink.
The living and dining room are neat and clean. The facility maintains a comfortable temperature at 73°F. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. There is a large television in the living room.
Fire extinguisher -There are three (3) fire extinguishers that are located in the kitchen area, hallway, outside /patio area observed to be fully charged and last inspected on 07/2025.
