Complaint Investigation Report
During interviews with residents, two (02) out of four (04) stated staff treat them with dignity and respect and one (01) out of four (04) residents stated S1 held a lighter to their face and asked called them stupid and is very rude but added that no one has ever physically abused them. LPA was unable to interview one (01) out of four (04) residents as they were out in the community during both visits.
Based on interviews, there is not enough information to verify the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time.
--- Staff did not provide resident with adequate food services.
It was alleged that residents do not like the food being served and staff say, “you eat what I make.” To investigate the allegation, on 03/06/2025 LPA conducted a physical plant tour at around 1:00p.m., requested pertinent documents at around 1:30p.m., interviewed one (01) staff from around 1:45p.m. - 2:15p.m. and two (02) out of four (04) residents from around 2:15p.m. - 2:45p.m. On 06/20/2025, LPA interviewed one (01) additional resident and one (01) additional staff from 11:00a.m. – 12:00p.m. During the physical plant tour, LPA observed a variety of fresh and frozen foods available for consumption. A review of the facility menu shows that a variety of well-balanced meals are served for breakfast, lunch and dinner. During interviews with staff, all staff stated they try their best to incorporate what they like, but it does not always work out because they sometimes argue and make demands. We try our best, but we meet our obligations as far as providing a nutritious meal. You can see from our menu that we offer a variety, but unfortunately, we cannot make separate meals for each client. During interviews with residents, two (02) out of four (04) residents stated although breakfast, lunch and dinner are served, they are not please with the amount of carbohydrates that are included at dinner time such as rice and pasta. One (01) out of four (01) stated the food is adequate. LPA was unable to interview one (01) out of four (04) residents as they were out in the community during both visits.
Based on observations, record review and interviews, there is not enough information to verify the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time.
No health and safety hazards noted during the visit.
