Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted a case management visit to correct the civil penalty assessment form issued on 9/20/24. LPA met with Administrator, JingFang Zhang, and explained the purpose of the visit.

On 9/20/24, LPA Chan issued the civil penalty assessment form with the incorrect date for the repeated violation. The visit today is to correct the date of the repeated violation.

An exit interview was held and a copy of the report. The LIC421FC was manually created and given to the administrator.
