Facility Evaluation Report

Licensing Program Analyst (LPA) Vaid conducted unannounced Physical Plant Inspection. LPA met with Administrator Gil Agas and discussed the reason for the visit. Last physical plant inspection was conducted on 12/10/24 with the current licensees knowledge.

Residential Care Facility for the Elderly to serve the Elderly for 60 years and older. The capacity is 200 which including 149 non-ambulatory residents and 15 may be bedridden. Facility has 25 residents with Hospice wavier.

LPA Vaid and House Manager Justin Lee toured the physical plant, did not observe any health and safety concerns.

Structure: The facility structure is located in a residential /business neighborhood, with 91 bedrooms, 91 bathrooms, 1 TV room, an administrative office, a restaurant style kitchen, a laundry room and a coin operated laundry room, a janitor storage room, an activity room, a dining room and two elevators. There is a covered patio area on the premises. Passageways, walkways, stairs and patios are free from obstructions. The entrance and side areas are free of hazards and debris.

Residents Bedrooms: Bedroom has a chair, nightstand, adequate lighting, adequate closet and drawer space. Bedrooms are spacious and allow for easy passage between and comfortable for usage of assistant devices such as walkers. The resident bedrooms are spacious and will easily accommodate the client's furnishings.

Resident Bathrooms: All bathrooms have a working toilet, wash basin, bathtub/shower. Bedrooms for non-ambulatory residents are accommodated for residents in a wheelchair. Skid mats and grab bars observed.
