Facility Evaluation Report

On today’s visit, Licensing Program Analyst (LPA) Vaid, Licensing Program Manager (LPM) Fierros met with Virgilio Agas, Administrator and Justin Lee, Manager. The purpose of this meeting was to discuss the licensee closure of existing management and transfer of current residents to the new management company.

LPA and LPM met with residents to discuss the change of licensee and management in the activities room

32 Residents were present at the meeting.

Resident Council President Lyndon Larson and Activity Director Nitrice Johnson were also present.

Notice of licensee change was reissued to residents on todays date 10/24/25. Regarding their rights during the change of licensee.

Residents may choose to remain in the facility or resident may choose to relocate to another location.

Many residents were concerned about the ALW program.

Residents were informed to pay the current licensee rent, and to speaks with the Care Coordinator Agency (CCA) representative about the rent and the new applicant.

Residents were informed the DHCS is aware of the closure and new tenant and to please follow up with the CCA regarding their ALW status/eligibility.
