On 6/29/2023 visit to facility during investigation of complaint, LPA observed deficiencies in resident's room.
1) Room #212 has gapping hole on tub outer sill.
2) Room #215 Baseboard in bathroom is unglued and needs repair or replacement.
3) Room #213 AC unit is not operable, front door lock is broken, and there appears to be water leak stains on the ceiling tile.
Deficiencies are being cited according to California Code of Regulations, Title 22, Division 6, Chapter 8 on LIC 809D.
An exit interview was conducted with Administrator, A hard copy of this report and appeal right were provided.
