Facility Evaluation Report
Licensing Program Analyst (LPA) Abeye Duguma met with Steven James for a Required One (01) Year visit. LPA explained the reason for the visit. A tour of the physical plant was conducted at around 11:00 AM and the following was noted:
There is one main entrance being utilized at the facility. The facility has a gated rear entrance for vehicles. The garage is being for parking and storage. The facility has a covered shaded area for clients and gardening activities. The facility does not have a swimming pool/body of water.
The facility is fire cleared for one hundred twenty (120) ambulatory of which twenty (20) may be non-ambulatory. The facility is currently providing center based services for twenty (20) clients.
The facility has five (05) activities rooms and three (03) restrooms. The bathroom was checked for cleanliness and proper operations. Personal hygiene product provided by the licensee. Hallways are well lit. The facility maintains a comfortable temperature at 77°F. The hot water temperature was measured at 117.3°F. The smoke and carbon monoxide detectors are hardwired, interconnected and connected to a centralized panel. Fire extinguishers are located throughout the facility, observed to be fully charged and last inspected 05/02/2025.
