On 11/08/2022, between 11:00am and 12:30pm, LPA reviewed facility files. The information contained in the files confirmed what staff had told LPA. LPA also read the investigative report from the Regional Center. The Regional Center's findings regarding this case were unsubstantiated.
Based on interview and record review there is not sufficient information to support this allegation. Therefore, this allegation is deemed UNSUBSTANTIATED at this time;
