Facility Evaluation Report
At 12:30 p.m., Licensing Program Analyst (LPA) Antonia Alvizar-Ettima conducted an unannounced annual inspection at the facility mentioned above. LPA met with Direct Support Professional (DSP’s). DSP Herrera contacted the Administrator, Juana Busby via phone and LPA discussed the purpose of this visit. Later Administrator joined us.
At approximately 1:00p.m., LPA and DSP Herrera conducted a physical plant tour inside and out. During the tour, LPA observed that the facility is a home located in a residential community. Required postings were observed in the living room area. The fire extinguishers are located throughout the facility with purchased date 05/18/26. Fire/ Disaster drill was last conducted on 03/03/26. Earthquake Emergency drill was last conducted on 03/01/2026. During this visit the facility is 72 degrees Fahrenheit. The facility is fire cleared for age range 18 through 59. Approved for six (6) ambulatory only.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Kitchen: LPA observed kitchen appliances and fixtures were functional. The kitchen has a working gas stove, faucet, freezer, refrigerator, and microwave. LPA found at least two (2) days perishable and seven (7) days non-perishable food at the facility that is properly stored. Frozen foods are wrapped, dated, and stored properly as well. Knives were stored in a locked drawer in the kitchen. LPA observed a working washer and dryer in the kitchen area. Next to the washer and dryer is a locked cabinet storing laundry detergents, cleaning agents and other toxins.
