Licensing Program Analyst (LPA) Alberto Lopez made an unannounced visit to deliver deficiency citation. LPA met with Rochelle Carpio Administrator and discussed purpose of visit.

During the investigation of complaint #28-AS-20250219161002 One resident showed inappropriate sexual behaviors on 01/28/2024, 03/08/2024, 03/09/2024, 04/27/2024, 05/01/2024, 05/06/2024, 05/09/2024, 06/26/2024, 08/10/2024. and 07/14/2024. Facility did not report these incidents to the department as required,
