Facility Evaluation Report

Licensing Program Analyst (LPA) Alberto Lopez conducted a subsequent unannounced Required - 1 year visit using the full Care Compliance and Regulatory Enforcement (CARE) Tools to complete inspection. LPA met with Anel Hernandez DSP and explained the purpose for the visit. The administrator arrived a short time later and assisted with the visit. The facility is approved to serve Developmentally Disabled Adults, four (4) ambulatory clients ages 18-59. One client is over 60. The facility is licensed as a level 2 home vendored by East Los Angeles Regional Center.

Staffing : There is sufficient staffing at the facility. Staff employed are over the age of 18, are fingerprint and cleared and associated to the facility.

Personnel Records-Training : Staff files are maintained at the facility. LPA reviewed staff files for four staff S1-S3. Staff have current CPR/first aid training and sufficient on-going training that meets the annual requirement. Staff have their Health Screening and Tuberculosis Screening on file. Staff are also trained on Abuse Reporting. Administrator Certificate expires 08/20/2026.

Client Rights-Information: Client personal rights and House Rules are posted. Per Facility Administrator, facility provides wi-fi services for facility clients.
