Facility Evaluation Report
Licensing program Analysts (LPAs) Alberto Lopez and Jewel Baptiste made unannounced visit to follow up on a client's death. On 03/23/2026 at approximately 11:00 am, C1 was seated in the dining room for lunch and Staff Johnny Bogen noticed C1 had a mouth full of food and immediately threw up. After staff cleaned and changed client, he was taken to TV room and after 20 minutes, staff approached client and noticed client was nonresponsive. Staff called 911 right away and administered CPR. When paramedics arrived, they continued CPR for about 30 minutes and pronounced client deceased after that. Police came and provided police report number to facility. LPAs took tour of facility and interviewed three (3) staff. LPA's noticed there were several non-ambulatory clients’ participating in the program, administrator confirmed they have three (3) non ambulatory clients. The facility is not licensed for any non-ambulatory,
Technical violation and appeal rights provided.
Administrator was asked to provided the required documents to get fire clearance for non ambulatory by 04/19/2026
