Licensing Program Analyst (LPA) Alberto Lopez made an unannounced visit to investigate one allegation. LPA met with Gabriela Cho, House manager and discussed the purpose of the visit.
During the investigation, LPA discovered that facility does not have admission agreements with the residents.
Facility did not have complete staff files at the facility during visit.
LPA advised to keep complete and current staff files at facility at all times,
LPA advised licensee to create Admission agreements for all 13 residents and send them to LPA within 10 days.
