Facility Evaluation Report
Licensing Program Analyst (LPA) Evelin Rios conducted an unannounced Case Management visit to this facility. The purpose of today’s visit was to serve the Order of Immediate Exclusion from Facility for Staff #1 (S1). LPA was greeted by staff and granted entry. At the time of visit LPA observed two (2) client and three (3) staff. Staff contacted the administrator via telephone and informed them LPA was at the facility. Administrator met LPA shortly after.
On today's visit LPA met with Administrator, Sarah Long and explained the reason for the visit. Administrator was served an Immediate Exclusion Order for S1. The Administrator informed LPA that S1 has never worked for the facility since they have been the administrator.
No immediate Health and Safety Hazard were noted during this visit.
Exit interview conducted. A copy of the report was provided to the Administrator.
