Facility Evaluation Report
On August 5 2026, Licensing Program Analyst (LPA) Sarah Benson arrived at the facility to conduct an unannounced case management. LPA met with care staff, and new Administrator John Sutterfield (cert #6084359735) and explained the purpose of the visit.
LPA Benson shared the CARE TOOL with the administrator. The administrator reported three of the five residents have returned from their summer break. LPA Benson interviewed all three residents.
LPA Benson and staff toured the facility and grounds together to ensure the health and safety of residents in care.
No citations were issued per Title 22 Regulations.
An exit interview was conducted and copy of the report left with the administrator.
