Facility Evaluation Report
11/19/2025 11:00 AM Licensing Program Analyst (LPA) Rebecca Knight arrived at the facility unannounced to conduct a Required-1 Year inspection. LPA met with licensee / administrator Carrie Lindeman ( 7002762735 exp. 08/03/2025 ) and explained the purpose of the visit.
LPA Knight and the administrator toured the facility together to ensure the health and safety of clients in care. Areas toured include but are not limited to two (2) client rooms, common areas, two (2) bathrooms, kitchen, storage areas. Staff and resident files were reviewed. All employees requiring background checks are cleared. Administrator certificate expired 08/03/2025, a citation was issued during the visit. Medication is locked in a cabinet. Medications were reviewed.
There is a schedule of recreational activities planned for the clients and activities are planned according to the clients preferences. Bedding, linens, and towels for clients were observed and found to be clean and in good repair.
The facility was observed to be at a comfortable temperature. Hot water measured between 105 – 120 degrees F. Common area, kitchen, and bathrooms were clean and in good repair. All bedrooms had required furniture, bedding, and lighting. Food appears to be stored and prepared properly. Facility has required (7) seven-day non-perishable and (2) day perishable supply of food. Fire extinguishers fully charged and inspected. Smoke detectors are all operational. There is a pool in an enclosed and locked pool room. The facility has been conducting emergency disaster and fire drills quarterly.
LPA has requested the following documents to update the facility files: Updated Surety Bond, LIC500 Personnel Report, LIC308 Designation of facility responsibility.
A deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22, and California Health and Safety Code. Failure to correct the deficiency may result in civil penalties. Exit interview conducted and copy of report was provided to licensee / administrator Carrie Lindeman.
