Facility Evaluation Report

At approximately 1:30 PM, Licensing Program Analyst (LPA) Julie Florio, Licensing Program Manager (LPM), Bethany Moellers, LPM, Kimberly Mota, Regional Manager (RM), Carla Nuti-Martinez, Lake County Behavioral Health, Lake County Public Guardian, Adult Protective Services, Local Long Term Ombudsman, and potential placement professionals for clients in care arrived unannounced to conduct a health and safety check and met with Mariah Uden, Administrator. Facility is an Adult Residential Care Facility (ARF) with five (5) clients in care.

The LPA and LPM observed the outside deck area free from obstructions. A tour of the facility was conducted. Behavior health and conservator are discussing relocation housing alternatives and agrees to update CCL if any clients in care relocate.
