Facility Evaluation Report
An office meeting was conducted today, 04/22/2026, in the Santa Rosa Regional Office. The following individuals were present in the meeting: Licensing Program Manager, Bethany Moellers, Licensing Program Analyst, Julie Florio, Licensee, Arlene Wing. The purpose of the informal office meeting was to discuss the current operation of the facility, fire clearance concerns, and to obtain proof of a current lease agreement .
On 04/01/2026, LPA Florio conducted a required 1-year annual inspection and observed household items, furniture, appliances, and additional items obstructing the walkways throughout the facility's porch.
During today's office meeting, Licensee was informed that these items shall be removed in order to bring the facility back into compliance with regulation. Licensee agreed. Additionally, LPA requested a copy of the facility's current lease agreement be submitted to the Department by EOB 04/24/2026. Licensee agreed.
No deficiencies were cited during today's office meeting.
