Facility Evaluation Report
At approximately 11:30 AM, Licensing Program Analyst, (LPA) Julie Florio arrived unannounced for the purpose of conducting a facility closure inspection. LPA was greeted by staff Carole Becker. Megan Kapitan, Administrator was contacted via telephone and arrived shortly after. Facility operated as a day program for adults with disabilities. The last day of operation was yesterday, June 30, 2025. There are no clients present in the facility today. LPA was informed that the facility will be used as office space for the corporations staff moving forward. There were a total of four (4) staff present during today's inspection. There were thirteen (13) clients attending the day program at the time the facility announced it's closure. The purpose of the inspection is to confirm the licensed program services have been discontinued and verify that there are no clients in care.
At approximately 12:00 PM, LPA conducted a walk-through of the facility with Administrator, inspected all rooms and the exterior of the building. Since the facility will continue to be used as office space for the corporations administrative personnel, there were supplies, office furnishings, and staff present. Administrator informed LPA that they no loner wish to operate a licensed day program due to decreased client population. Administrator states that the clients were offered the option of transferring to the sister day program in Clearlake or to one of their community based programs here in the County. Since the clients were high functioning, they transferred to the community based programs. LPA confirmed that the Department received email notification of the facility's intent to close on May 9, 2025 with a copy of the letter that was sent to the clients in care and their families.
Closure inspection of this facility has been completed. Facility surrendered the license to LPA. The Department will move forward with the closure process. LPA to finalize paperwork for final closure.
No deficiencies cited during today's visit.
