Facility Evaluation Report

License Program Analyst (LPA) Hansen arrived unannounced to conduct a Required 1-year Inspection of the facility. LPA was welcomed by Licensee Jennifer Fitts & Licensee/Administrator Cory Ferris.

At approximately 8:45 AM, LPA toured the building and grounds which was found to be in good repair, although clients bedrooms and bathrooms had strong odor with dust, cobwebs and clutter. LPA observed all walkways and exits to be unobstructed. The amount of fresh and non-perishable foods was within regulation. Toxins are secure and not accessible to clients. Medication is centrally stored and secure in locked kitchen cabinet. There is a sufficient supply of hygiene products and linens on hand for client use. Hot water temperature measured 116.6 degrees F. within Title 22 acceptable regulation of 105 to 120 degrees F in client’s bathroom. Fire extinguisher was last inspected 2/16/2023. Smoke alarm and Carbon Monoxide detector were tested and in working order, facility has installed new fire and alarm monitoring system that will alert their phones and fire department. Disaster Drills are conducted twice yearly, with the last drill conducted 11/18/2023. Facility does not handle client monies. Facility had some water damage in 2023 to the roof, contractors have fixed although ceiling beam from kitchen to living room still has water damage and facility is in the process of getting repaired.

At approximately 10:00 AM, LPA reviewed 3 of 3 Client records and 2 of 2 Staff records, which were all found to be thorough and contained the required documentation. First aid certification was current in staff files reviewed. Facility does not handle client monies. Administrator's Certificate was current.

No deficiencies cited during this visit in the areas inspected .

LPA Hansen obtained requested annual documents at visit.
