On 8/13/25, Licensing Program Analyst (LPA) J. Leffall conducted a case-management visit for a closure walk-through of the facility. LPA met with Administrator Debra Flores.
Licensee Margaret Jaurez informed the Department of her intent to forfeit the license and close the facility. LPA confirmed 8/13/25 visit that there are no residents who require care and supervision at the facility.
Exit interview was conducted and a report was provided.
