Facility Evaluation Report
On 8/13/25 Licensing Program Analysts (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced himself, stated the purpose of the visit, and was greet by Sheila Cox Lindsey, Licensee (L1). LPA was granted entry. 4 clients were present during inspection.
LPA toured facility with L1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Medications were checked and observed kept locked in medication cart in the office. Clients’ MARS was not reviewed. Licensee does not have a current MARS in facility. LPA was unable to verify signing of medications. Cleaning chemicals was observed stored and locked in facility office. Fire extinguisher was observed one in each building with a service date of: 4/24/25. Fire drill last completed on 7/2/25. Clients' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature range of 105.4 to 105.9 in 2 bathrooms.
Outside of facility toured. Outside observed free of debris. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained range at 38 degrees F. Smoke detector and Carbon monoxide were tested and observed to be operational. All clients’ files reviewed and 1 document was missing. Samples staff files were reviewed to have all required documents. Technical Support Services recommended and accepted by Licensee.
No deficiencies issued during this inspection.
Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 8/27/25: Lic 308, Lic 400, Lic 402, Lic 500, Lic 610D, and Lic 9020. A copy of this report with 9102 was provided to Licensee, whose signature on this form confirms receipt of this report.
