Facility Evaluation Report
On 10/22/24 at 9:11am Licensing Program Analysts (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced himself, stated the purpose of the visit, and was greeted by Sheila Cox Lindsey, Licensee (L1). LPA was granted entry. 5 clients were present during inspection.
LPA toured facility with L1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. A sample of resident’s medications were checked and observed kept locked in medication cart in the office. Clients’ MARS were reviewed. Cleaning chemicals was observed stored and locked storage area. Fire extinguisher was observed with a service date of: 4/10/24. Fire drill last completed on 10/7/24. Clients' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested 120 degrees F in bathroom 1, 115 degrees F in bathroom 2.
Outside of facility toured. Outside observed free of debris. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Freezer temperature was maintained range at 0 degrees F and refrigerator temperature was maintained range at 37 degrees F. Main kitchen refrigerator and freezer did not have thermometer. Carbon monoxide were tested and observed to be operational. All clients’ files reviewed to have all the required documents. Samples of staff files were reviewed to have all required documents.
No deficiencies issued during this inspection. Technical Advisory assigned to Licensee. A copy of LIC 9102 issued to Licensee. Licensee to submit photos of thermometer and receipt of purchase to CCLD.
Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 11/5/24: Lic 308, Lic 400, Lic 402, Lic 500, and Lic 610D, . A copy of this report was provided to Sheila Cox Lindsey L1, whose signature on this form confirms receipt of these reports.
