Facility Evaluation Report

On 3/23/26, Licensing Program Analyst (LPA) J. Leffall conducted a case management visit to follow up and confirm details of an incident report that was received by the Department. The incident occurred on 2/27/26, in which Client’s Health and Safety were at risk where client was left in the van for an hour and a half. Staff was transporting clients to their residence and C1 was left in the van. An internal investigation was conducted, and it was concluded that Staff (S1) was at fault and left the client in the van from 3:30pm-5:00pm.

Per Internal Investigation and findings, S1 was terminated effective 3/2/26.

A citation is issued on the attached 809-D regarding client’s Health and Safety.

A copy of this report was provided to Program Director which confirms the receipt of this report.
