The inspector’s account

On 8/5/26, Licensing Program Analyst (LPA) J. Leffall conducted a case-management visit for a closure walk-through of the facility. LPA met with Licensee Zoria French.

Licensee informed the Department of her intent to forfeit the license and close the facility. LPA confirmed on the 8/5/26 visit that there are no residents who require care and supervision at the facility.

Exit interview was conducted and a report was provided.
