Facility Evaluation Report

On 07/26/24 Licensing Program Analysts (LPA) M. Yang and J. Leffall arrived unannounced to conduct an Annual Inspection. LPAs introduced themselves, stated the purpose of the visit, and was greeted by Jill Marmon, Administrator (A1). LPAs was granted entry. 13 clients were present during inspection.

LPAs toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Medications were checked and observed kept locked in medication cart in the office. Clients’ MARS was reviewed. Cleaning chemicals was observed stored and locked in facility office. Fire extinguisher was observed one in each building with a service date of: 12/13/23. Fire drill last completed on 7/18/24. Clients' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested 116.2 in separate bathroom, 111.7 in downstairs bathroom, and 114.4 in upstairs bathroom.

Outside of facility toured. Outside observed free of debris. Side gate was self-closing. Outside was observed with adequate outdoor seatings available for clients. Freezer temperature was maintained range at -5 and 0 degrees F and refrigerator temperature was maintained range at 39.6 to 40 degrees F. Carbon monoxide was tested and observed to be operational. A sample of clients’ file reviewed to have all the required documents. A sample of staffs’ files were reviewed.

No deficiencies issued during this inspection.

Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 08/2/24: Lic 308, Lic 400, Lic 402, Lic 500, Lic 610D, and Lic 9020. A copy of this report was provided to Jill Marmon A1, whose signature on this form confirms receipt of these report.
