The inspector’s account

On 08/22/23, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA met with House Manager Linda Aguilar. LPA introduce self, stated the purpose of the visit and request to met with Administrator. House Manager was unable to reach Administrator. LPA conducted tour with House Manager. All 10 residents were present upon LPA arrival. 5 clients depart to day program during inspection. Administrator Jill Marmon arrived later during tour.

The facility was observed to be at a comfortable temperature of 72 degrees F in modular house and 75 degrees F in main house. The facility was observed clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and non-perishable food was observed. Temperature maintained for refrigerator range between 32 and 37 degrees F and freezer maintained at 0 to -10 degrees F. Fire extinguisher was observed with a service date of: 12/20/22. Fire drill last completed: 08/4/23. All bedrooms were observed to have required furnishings and with adequate lightening. Bathrooms were properly equipped, and the hot water temperature was tested at 111.2 degrees F in the modular house bathroom. Hot water was tested at 109.6 in the main house bathroom 1 and 119.4 degree F in the main house bathroom 2. Carbon monoxide and smoke detectors were tested and observed to be operational in both houses. Outside of facility toured and observed to be free of debris. Medications observed kept locked in medication cart in the facility office. MARs were reviewed. Cleaning supplies observed locked in facility office. A sample of the client’s file reviewed to have update emergency contacts, Admission agreement, and Pre-Appraisal. A sample of the staff files were also reviewed. Staff files were observed to have current First Aid, fingerprinted clear and associated to the facility.

No deficiencies issued during this inspection. Exit Interview conducted. A copy of current Administrator Certificate was received. The following documents are requested and submitted to Fresno CCL by: 08/28/23. Forms requested: Lic 308, Lic 400, Lic 405, Lic 500, and Lic 610D. A copy of this report was provided to Administrator, whose signature on this form confirms receipt of this report.
