On 1/22/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management visit to follow up on a incident involving Resident (1) that occurred on 12/20/2025.
Currently, there are 2 residents in placement. Resident 2 (R2) was a day program and resident (3) was at a doctor's appointment. During the case management visit, LPA toured facility, and conducted interviews.
