Facility Evaluation Report

Licensing Program Analyst (LPA) Shawna Doucette and Daiquiri Boyd arrived at the facility unannounced to conduct the Required Annual Inspection. LPA met with Program Director Angie Sarabia. LPA disclosed the purpose of the inspection and was granted entry into the facility by Program Director Angie Sarabia.

LPA Boyd conducted a tour with Program Director. The facility was set at 76 F temperature and free of passageway obstructions inside and outside. The water temperature was measured at 116.2 F.

Kitchen was toured. Cleaning supplies were in a locked storage room. Smoke detectors and carbon monoxide detectors were checked and operating. Fire extinguishers were charged and had service dates of 03/28/25. Fire drill was last completed on 07/23/25 and earthquake drill on 08/01/25..

LPA Doucette reviewed client and staff records. Current first aid and CPR were on file for staff. Facility does not administer medications or serve food.

An exit interview was conducted with the Program Director and a copy of this report was provided. .
