Facility Evaluation Report

Licensing Program Analyst (LPA) Shawna Doucette and Brianna Miranda conducted a Case Management for an incident that occurred on 10/10/24.

LPA's were notified during the 10/10/24 visit, S1 was using R1's credit card information to order Door Dash. LPA's were informed R1 was not at the facility when these transactions were made and delivered to the facility. During the 10/10/24 visit, Ridgecrest Police Department was contacted and a police report was filed.

After conducting an internal investigation, facility Administrator was able to determine S1 was responsible for ordering Door Dash using R1's credit card information. Administrator provided copies of the transactions made by S1 on R1's account.
