Staff spoke inappropriately to residents in care

Staff did not allow residents to leave the facility

On 06/16/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an initial complaint investigation. LPA met with Administrator Wendy Lee and stated the purpose of the visit. LPA delivered complaint findings on the above allegations during visit.

During the course of the investigation, records were reviewed, the facility was toured, and interviews were conducted. Residents confirmed staff speaks to residents nicely and in a gently voice. Residents were allowed to leave the facility supervised by staff and unsupervised according to physician’s order. Based on interviews conducted and records reviewed, the preponderance of evidence standard has not been met, therefore, the above allegations are found to be UNSUBTANTIATED. Exit interview was conducted. A copy of this report was provided to Administrator, whose signature on this form confirms receipt of this report.
