Facility Evaluation Report
On 08/19/25, Licensing Program Analyst (LPA) L. Salazar arrived at the facility unannounced to conduct the required annual inspection. LPA was greeted by Program Director, stated the purpose of the visit, and was allowed entry into the facility.
LPA toured the facility inside and out. Facility is considered Career Exploration program. LPA observed the required hand washing signs in client restrooms. An updated Infection Control plan was received prior to LPA's inspection. LPA observed the facility to be clean and free from odor. Facility temperature measured at 72 degrees F. Disinfectants and cleaning supplies were observed to be locked in closet inaccessible to clients. Water temperature was observed at 107 degrees. Facility disperses PRN medications. There are no participants with Restricted Health Care plans.
Emergency disaster plan and procedures are in place. Last fire drill was conducted on 08/18/25. Doors and passageways were observed to be free from obstruction throughout the program. Fire extinguishers were observed with a service date of 09/24/24.
A sample of client files were reviewed and observed to have Admission agreement, Needs and Service Plan, and annual Medical Assessment. A sample of staff files were also reviewed. Staff files were observed to have 8 hours of documented training and current First Aid/CPR certifications. Staff present at today’s visit were fingerprinted clear and associated to the facility. Quarterly Emergency Disaster Drill logs were observed for all staff. Last drill was an earthquake drill conducted in 07/28/25.
LPA obtained the following updated forms at the time of visit. Designation of Facility Responsibility (LIC308), Personnel Report (LIC 500), Client Roster (LIC 9020), and Emergency Disaster Plan (LIC 610D).An exit interview was conducted with Program Director. A copy of this report was discussed and provided at the time of visit. No deficiencies cited on today's visit.
