Facility Evaluation Report
On April 22, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a case management visit for an incident that happened on April 15, 2026. LPA met with Angela Vazquez Administrator.
LPA reviewed C1's physician report dated November 3, 1995, admission agreement dated March, 2, 2000 (different address listed), and current IPP (November 18, 2025). On the day of the incident April 15, 2026 the census was 35 and number of staff on duty was 17, which includes 2 Admin. At the time there were 2 of the 35 clients that require 1:1.
Administrator stated the surveillance shows C1 left out the door facing Jefferson St at 10:10 am, and staff located C1 at 10:18 am. C1 was able to leave the facility unassisted and without staff initially noticing.
LPA obtained a copy of current IPP, physician's report, and admission agreement.
Deficiencies observed were cited during today's visit, per California Code of Regulations, Title 22.
