Facility Evaluation Report

On 05/23/2025, Licensing Program Analyst (LPA) L. Salazar arrived at the facility unannounced to conduct the annual required visit. LPA was greeted by the Administrator, stated the purpose of the visit and was allowed entry into the facility.

LPA toured the facility inside and out with Administrator. An updated Infection Control plan was received prior to LPA's inspection. LPA observed the facility to be clean and free from odor. Facility temperature measured at 74 degrees F. Disinfectants and cleaning supplies were observed to be locked in a janitor closet and inaccessible to clients. Water temperature in the kitchen and bathrooms measured at 119.6.

LPA toured the facility with Administrator and observed clients in care to eating lunch in their classrooms. Facility was observed with 4 classrooms , 2 activities rooms, 6 bathrooms, kitchen and a conference room. Facility was recently remodeled with a capacity increase. LPA printed a current updated license at the time of visit. LPA observed there to be 8 program specialists present at the facility. First Aid kit observed in each classroom and in the kitchen. Last fire drill was completed 04/27/2025.

Emergency disaster plan and procedures are in place. Emergency numbers were observed to be posted next to the facility sketch. Assembly points and water shut offs were observed on the sketch. Last fire drill was conducted on 5/2024. Doors and passageways were observed to be free from obstruction throughout the program. Fire extinguishers were observed with an expiration date 04/23/24. Client and staff files will be reviewed at a later date.

LPA requested the following updated forms to be faxed to CCLD by 5/30/25: Designation of Facility Responsibility (LIC308), Administrative Organization (LIC309), Personnel Report (LIC 500), Client Roster (LIC 9020) that identifies which individuals resident in an Intermediate Care Facility (ICF's), and Emergency Disaster Plan (LIC 610D). No deficiencies cited on today's visit.
