Facility Evaluation Report
On 07/09/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an required Annual Inspection. LPA met Administrator (A1) Mario Bolanios and Assistant Program (S1) Director Brenda Rodriguez. LPA toured facility with S1.
LPA toured the facility inside and out. Facility was observed to have 4 activity rooms (3 classrooms & 1 kitchen) and 4 bathrooms. Meals are not prepared at the facility. All clients bring their own meals to the facility. LPA observed 13 staff members present at the facility. First Aid kit observed with required items. All bathrooms toured. Hot water temperature measured between 108.1 to 118.6 degrees F. First Aid kits were observed to have the required items. Client lockers were observed in activity room.
Cleaning supplies were observed to be locked in the closet hallway next to the office and hall shelf. Smoke detector and carbon monoxide detector were present in the kitchen. Sharps observed locked in kitchen closet. Last fire drill was conducted on 6/18/2026. Doors and passageways were observed to be free from obstruction inside and outside. Fire extinguishers were observed throughout the facility with a service date of 04/24/26. Client and staff files will be reviewed at the main office. A sample of client files and sample of staff files were also reviewed to have required documents.
Exit Interview conducted. LPA received copies of Lic 308, Lic 500, Lic 610D and Lic 9020 during visit. A copy of this report was provided to the Administrator, whose signature confirms received this report.
