Facility Evaluation Report
On 08/05/2025, Licensing Program Analysts (LPAs) L. Salazar and M.arrived at the facility unannounced to conduct the annual required visit. LPA was greeted by the Program Manager (PM), stated the purpose of the visit and was allowed entry into the facility. Administrator on record is Mario Bolanos, ARF Certificate # 7033547735 and RCFE Certificate #7033547740.
LPAs toured the facility inside and out. Facility was observed to have 4 rooms (3 classrooms & 1 kitchen) and 4 bathrooms. Floor in the Multi purpose room was observed to have cracks in the tiles and uneven in some areas potentially causing a tripping hazard. LPA observed 14 staff members present at the facility. First Aid kit observed. Water temperature measured at 109 degrees. First Aid kits were observed to have the required items.
LPA observed the facility to be in need of deep cleaning and painting on and around the doors/doorframes. Floor located between the bathrooms near the exit door is uneven and cracked and needs replacement. Facility was free from odor. Facility temperature measured at 74 degrees F. Disinfectants/cleaning supplies were observed to be locked in the closet hallway next to the office. Smoke and Carbon Monoxide detector were present in the kitchen.
Emergency disaster plan and procedures are in place. Last fire drill was conducted on 5/14/2025. Doors and passageways were observed to be free from obstruction. Fire extinguishers were observed with a service date of 06/16/25. Client and staff files will be reviewed at the main office and documented on an annual continuation visit.
LPA requested the following updated forms to be faxed to CCLD by 08/15/25: Designation of Facility Responsibility (LIC308), Administrative Organization (LIC309), Personnel Summary Report (LIC 500), Client Roster (LIC 9020) and Emergency Disaster Plan (LIC 610D). No deficiencies cited on today's visit.
