The inspector’s account

On 07/20/2026, Licensing Program Analysts (LPA) J. Duarte arrived at the facility unannounced to conduct the annual required visit. LPA was greeted by Administrator (AD) Alicia Lopez. LPA explained the reason for today's visit and toured the facility with AD.

LPA observed the facility to be clean and free from odor. Facility temperature measured at 78 degrees F. Disinfectants and cleaning supplies were observed to be locked in a utility closet inaccessible to clients. First Aid kits were observed to have the required items. The facility has a movie room, LPA observed clients watching a movie. The facility also has two multipurpose rooms and LPA observed clients watching television, playing on tablets, sitting on chairs, making puzzles, and observed staff walking a client. The facility a laundry room to wash client clothing protectors that are used at lunch time, wash cloths to clean tables, and to wash sheets that are used on the sitting matts.

The facility has a lunch room and keeps sharps stored in a locked kitchen cabinet. The kitchen has a fridge to store staff lunches. AD stated that clients bring their own lunch or purchase lunch on outings. The facility does not store medication or administer medication to clients.

The facility has five restrooms. The hot water temperature in the restrooms measured between 105 and 109 degrees F.
