Facility Evaluation Report
Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to conduct a Case Management. LPA met with Facility Director, Jocelyn Kosgeryan, and explained the purpose of today's visit.
Staff 1 does not have required First aid / CPR training.
Staff 1, staff 2, and staff 3 does not have required Health Screening /TB test in file available for review.
The facility did not report Kern Regional Centers investigation into multiple facility staff for inappropriate behavior toward clients.
The following deficiencies are being cited per Title 22 regulations.
