Facility Evaluation Report
On 03/18/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and requested to meet with Administrator Antwanique Walker who stated has been the administrator for the facility since August 2025. Two clients were present during inspection.
The facility was observed to be at a comfortable temperature clean, in good repair, and no passageway obstructions or fire hazards were observed inside. Fire extinguisher was observed with a service date of: 01/22/26. Fire drill last completed on 02/03/26. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 40 degrees F. Cleaning chemicals was observed stored and locked in the garage and under kitchen sink. Sharps observed locked in kitchen drawer. Medications were checked and observed kept locked medication cart. Clients’ MARS was reviewed. Washer observed operating during visit. Clients' bedrooms were toured. Bathrooms are toured. Hot water temperature was tested 105 degrees F.
Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seating available for clients. Carbon monoxide was tested and observed to be operational. Facility has fire sprinklers installed throughout the facility. All clients’ and sample of staff files were reviewed.
A deficiency is being cited on the attached 809D in accordance to California Code of Regulations, Title 22, Division 6. Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 03/24/26. Forms requested: Lic 308, Lic 500, Lic 610D, and Administrator certificate. A copy of this report and appeal rights was provided to Administrator whose signature on this form confirms receipt of this report.
