The inspector’s account

On 04/08/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and requested to meet with Administrator. LPA met with program supervisor Regina Browden. LPA contacted Administrator Lester Manigque via telephone and left voice message. LPA toured facility with Program Supervisor. Designee Brandy Frazier arrived shortly during tour. Three clients were present during inspection. Area Director Donte Williams arrived later during inspection.

The facility was observed to be at a comfortable temperature clean, in good repair, and no passageway obstructions or fire hazards were observed inside. Fire extinguisher was observed with a service date of: 01/21/25. Fire drill last completed on 03/20/25. An adequate supply of perishable and non-perishable food was observed. Expired non-perishable food and perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 40 degrees F. Clients' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured. All bathrooms observed toured. Hot water temperature was tested 105 degrees F. Medications were checked and observed kept locked medication cart. Clients’ MARS was reviewed. Cleaning chemicals was observed stored and locked in the garage and under kitchen sink. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Carbon monoxide and smoke detectors were tested and observed to be operational. All clients’ and sample of staff files were reviewed to have all the required documents.

A deficiency is being cited on the attached 809D in accordance to California Code of Regulations, Title 22, Division 6. Exit Interview conducted. LPA received copy of Lic 500, and Lic 9020. Current Administrator Certificate and Lic 610D is requested and to be submitted to Fresno CCL by 04/15/25. A copy of this report and appeal rights was provided to Designee, whose signature on this form confirms receipt of these report.
