Facility Evaluation Report
On 10/24/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA introduce self, stated the purpose of the visit and met with LPA Yang met with Licensee (L1) Felipe Curran who granted LPA entry into the facility. LPA toured the facility with L1. There are no residents currently reside at the facility.
The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. All residents’ bedrooms were toured and observed to have the required furnishings and with adequate lighting. An adequate supply of perishable and non-perishable food was observed. Refrigerator temperature is maintained at 40 degrees F and freezer at -4 degrees F. Fire extinguishers was observed throughout the facility with service date 11/07/24. Kitchen was toured and observed to have dishes, plates, and utensils. Refrigerator temperature maintained at 32 degrees F and freezer temperature maintained at -5 degrees F. LPA observed adequate food supplies. LPA observed an extra supply of bed linens, towels, and personal hygiene products. Bedrooms were observed to have the required furnishing and are ready for occupancy. Bathrooms were tour. LPA observed grabbed bars and non-skid mats. The bathrooms were toured. Non-skid mat and grabbed bars were observed. Hot water temperature tested ranged between 116.2 and 117.7 degrees F in hall bathroom and ranged between 116.2 and 115.4 degrees F in master bathroom. Outside of facility toured. Adequate outdoor seatings available for resident. Swimming pool fenced observed locked. LPA observed exit side gate was observed free of debris. Smoke detectors and carbon monoxide were observed to be operational during this inspection.
No deficiency during visit. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 10/30/25: Lic 308, Lic 500, Lic 610E, and current liability insurance. A copy of this report and appeal rights was provided to Licensee, whose signature on this form confirms receipt of this report.
