COMPLAINT CONTROL NUMBER: 24-AS-20250514164938
Staff do not prevent resident from engaging in self -harming behaviors while in care.
Licensing Program Analysts (LPA) Sarah Hurt and Shawna Doucette conducted an unannounced facility visit to deliver findings on the allegations listed above. LPA met with facility Administrator, Nancy Cudal, and explained the purpose of today's visit.
Regarding the allegation Staff do not prevent resident from engaging in self -harming behaviors while in care. LPA's observed Resident 1 on two seperate visits to the facility on 05/21/2025, and 07/17/2025. During the visit on 05/21/2025 Resident 1 was clothed, and appeared to be clean. Resident 1 stated during an interview on 05/21/2025 that they are doing well at the facility and they like living here. Resident 1 was in the dining area on 07/17/2025 and they stated they are doing well at the facility and had just finished eating lunch which included sandwich, salad, and watermelon. On 07/17/2025 Resident 1 appeared to be well dressed including shoes. LPA's observed Resident 1's room to be tidy with required furnishings and bedding made nicely. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
No deficiencies cited today Per title 22 Regulations.
